An oxazolidinone that inhibits protein synthesis. Used for resistant Gram-positives where Vancomycin fails or is not tolerated. Bioavailability: 100% (IV dose = Oral dose). Excellent tissue penetration (Lung/CNS).
indications
- VRE (Vancomycin Resistant Enterococcus): First line.
- MRSA Pneumonia: Superior lung penetration compared to Vancomycin.
- Necrotizing Fasciitis: Toxin suppression (theoretical benefit).
adverse effects
- Myelosuppression: Thrombocytopenia/Anemia. Risk increases after 14 days. Weekly CBC required.
- Serotonin Syndrome: Weak MAO inhibitor. Caution with SSRIs/TCAs/Fentanyl.
- Neuropathy: Peripheral/Optic neuropathy (long term use).
- Lactic Acidosis: Mitochondrial toxicity.
